More information about immunizations, illness, absences,
etc., and necessary forms can be found at the MNSD
Health Services website!
A. You are required to provide proof of immunization - including:
1) 4 DPT - one given on or after the 4th birthday.
2) 3 Polio - (a fourth may be required soon).
3) 2 Measles and at least 1 Mumps and 1 Ger. Measles
(preferably given as 2 combination MMR shots).
4) 3 properly spaced Hepatitis B; this series is now required
of all students in grades K, 1-5 and 7th grade in 2002-2003.
5) New for original entry and for 7th grade in 2002 - 1 Varicella
(Chicken Pox/varivax) or written (parent or physician) statement
of disease.
B. A Physical Exam is required given within 1 year of entry to
school in Kindergarten or grade 1, and for transfer students.
C. A Dental Exam is required given within 1 year of entry to
school in Kindergarten or grade 1, and for transfer students.
Notice to the parents of 3rd and 7th grade
students:
A Dental Exam is required within 1 year of entry to grade 3 and
grade 7. A form is available from the school nurse's office, and
one will be sent to you at the end of grades 2 and 6.
Notice to the parents of 6th and 11th grade
students (2002-2003):
A Physical Exam is required within 1 year of entry
to grade 6 and grade 11. A form is available from the school nurse's
office, and one will be sent to you at the end of grades 5 and
10.

physician's attention on the occasion of your next visit.
