School Health Program — General Information . . .

IMMUNIZATIONS — See immunizations page for detailed information on immunizations required by law.

TUBERCULOSIS TEST — A TB test may be required, at school nurse's discretion.

SCOLIOSIS SCREENING — Scoliosis screening is performed in grades six and seven.

VISION SCREENING— Vision screening is performed in kindergarten through 12th grade and as needed.

HEARING SCREENING — Hearing screening is performed in grades kindergarten, one, two, three, and seven, eleven and as needed.

HEIGHT, WEIGHT and BMI — Height and weight measurements are obtained periodically, and as mandated by the state. In addition, the State requires that parents be notified of a Body Mass Index (BMI), a ratio of height to weight for age, that indicates that a student may be at risk for obesity. In that event, parents will be advised to consult with their Physician.

PHYSICAL EXAMINATIONform may be downloaded here — Students must have a complete physical examination upon the first day of original entry into school (kindergarten or first grade), grade six and grade eleven and at the nurse's discretion. Students new to Pennsylvania schools must have a physical examination if the student’s record does not meet Pennsylvania requirements.

DENTAL HEALTH REQUIREMENTSform may downloaded here — All children are required to have a dental examination upon original entry into school (kindergarten or grade one), and in grades three and seven. Dental screening may be performed on an as-needed basis.

Parents are encouraged to take children to their family physician and dentist for examinations to provide continuity of care. Family physicians and dentists are familiar with your child and thus are better able to detect physical/dental changes. He/she is able to begin immediate treatment if indicated and to follow through with needed care and immunization boosters. Families are provided with appropriate forms when examinations are required.

If financial need is demonstrated, the Children’s Health Insurance Program (CHIP) offers low cost medical insurance. In special cases, the school physician/dentist may perform the required examination. Contact the nurse at your school for further information.
