INTRAMURALS
SPORTS PHYSICALS
Prior to any student participating in try-outs, practices, scrimmages or contests, the student is required to complete a physical evaluation.
Students should complete a COMPREHENSIVE INITIAL PRE-PARTICIPATION PHYSICAL EVALUATION (CIPPE), which is valid for one calendar year unless the student suffers a serious illness or injury within those twelve months.
Parents/Guardians must make their own arrangements with their family physician in order to take advantage of the CIPPE. Also, you must use the CIPPE form that is attached and complete all of sections 1, 2, and 3, as well as sign where it is required. In addition, the top two lines of section 4 must be completed. Your examining physician will complete the remainder of section 4. The student's personal physician must have a verified signature by having the doctor stamp the CIPPE form with his office stamp, or by signing and attaching an Rx slip.
Please turn the CIPPE form in to the Paxon Hollow Middle School Athletic Director so he may keep track of the time frame that your exam will cover.
PHYSICAL RE-CERTIFICATION
Comprehensive physicals using the CIPPE form are good for one calendar year; however, students must be re-certified when playing a subsequent sport. Re-certification means parents must complete Section 5 of the CIPPE. Section 5 asks several questions regarding the student’s health history since their physical exam. If all of the health history questions are answered no, Section 5 will re-certify the student for participation. If any health history question on Section 5 is answered yes, the student must see a Doctor and turn in Section 6 to be re-certified for participation.
PLEASE TAKE NOTE THAT THE P.I.A.A. REQUIRES:
A PHYSICAL EXAM BEFORE AN ATHLETE CAN BEGIN TO PRACTICE, INCLUDING TRY-OUTS.
THAT NO PHYSICAL EXAM TAKE PLACE BEFORE SIX (6) WEEKS PRIOR TO THE FIRST PRACTICE FOR EACH SEASON.

SPORTS OFFERED ARE:
Fall: 7th and 8th grade boys' soccer, 7th and 8th grade girls' soccer, 7th and 8th grade volleyball, 115 lb. football, unlimited weight football, 7th and 8th grade field hockey.
Winter: 7th grade boys’ basketball, 8th grade boys’ basketball, 7th grade girls’ basketball, 8th grade girls’ basketball, and wrestling.
Spring: 7th and 8th grade baseball, 7th and 8th grade softball, 7th and 8th grade lacrosse, 7th and 8th grade lacrosse, boys and girls track.
